Our CHD ECHO program

Hole in the Heart has collaborated with Project ECHO to create the first CHD (congenital heart defect) ECHO program forming partnerships between CHD experts and primary providers to help detect, diagnose, and create a continuity of care for CHD patients.

The seven (7)-session (once a month) program will provide knowledge ranging from neonatal – children – young adult to adult CHD care. The program will end with focusing on mental health awareness. The CHD ECHO program will help create mentorships and provide support to the medical community and the families they serve.

Each session will include a brief didactic presentation followed by a CHD patient case study with an interactive best practice care discussion and will offer a whole-person approach to CHD-related care.

Founder of Hole in the Heart, she is dedicated to help prevent other New Mexico families from experiencing childhood chronic illness and advocating for best practices for the detection and diagnosis of congenital heart defects.

M. Abigail Simmons, CHD Experts Coordinator

M. Abigail Simmons, MD, is a board certified congenital cardiologist who specializes in treating congenital cardiovascular disease in adult patients. Dr. Simmons can diagnose and treat structural heart disease, arrhythmias and acquired heart problems. She has a special interest in the adult congenital heart disease patient, with experience caring for those patients as they transition from childhood cardiology care.

We encourage primary care providers to present their own patient case study from either a current CHD patient or past patient with a CHD concern. Please complete the case study form BELOW and submit to CHDECHOprog@gmail.com for review.
